Power of Attorney, Living Wills &Trust Documents

$10 per notary stamp plus $25.00 service fee

Minimum service fee of $25.00.  Additional travel time will be charged if more than 8 miles from home office at a rate of .65 per mile.  NO FEES for initials!

Hospital visits are an option at a higher service fee.  If the patient is not fully alert or has been medicated, I will not be able to notarize the document(s). Please call for a quote

Administering Oaths & Affirmations

$46.00 Service Fee 1st 1/2 Hr

$10.00 Notary Fee if a document needs notarization. Additional travel time will be charged if more than 8 miles from home office at a rate of .65 per mile. Court witness telephone hearing is an additional $36.00 per 15 min if there is wait time before being excused by Judge.  Please call and get a personalized quote.

Attesting to a Photocopy

$10 per box plus $16.00 service fee per 15 minutes

A Few Tips

Each circumstance can be different.

​

 

Except for your signature(s) & initials, the document should be fully completed before my arrival, to avoid additional service charges for having to wait for the document to be completed. 

Travel:  my fee is (.65 cents) per mile from my home office 

 Wait time is charged in 30 minute increments at a rate of $16.00 per with a minimum charge of $16.00. Wait time includes review of documents, and final review of accuracy 

 

Witnesses and Identification

Please have your witness(s) readily available

Proper Identification is MANDATORY for all parties!  

 

If you are in need of witnesses we have some available for an addtional fee.
